Clinical Evaluation on Surgical Treatment of Mitral Stenosis

Abstract
Clinical evaluation was made on a total of 22 cases, which received mitral commissurotomy in Chonnam University Hospital in the period from 1962 to 1968.
Patients age was distributed between 11 and 43 year old with the highest incidence in the age group of 2. to. 3. decade, sex ration of male to female was 2 : 1.
The chief subjective symptoms of the patients were exertional dyspnea and palpitation in 100% of the total cases, and then chest pain, bloody sputum, orthopnea, edems of extremities in order.
The physical findings were diastolic murmur at the apex (100%), diastolic and systolic murmur at the apex (36%), and opening snaps (45%).
The main radiographic findings were enlarged heart of pulmonary vascularities and left atrium(100%), congestion in the lung (64%), Kerley¢¥s B-line (14%).
ECG findings were normal sinus rhythm (73%), and atrial fibrillation (27%).
The operating methods was only digital commissurotomy in 16 cases and combination of digital procedure and Bailey¢¥s Valvulotom in 6 cases.
The result was excellent in 55%, good in 31%, poor in 9% and death in 5%. the cause of death was due to cerbral hyperthermia.
The mean pressure of the left atrium was 411 mmH©üO before commissurotomy and 285 mmH©üO after commissurotomy. Therefore, average drop of 126 mmH©üO was obtianed.
